Please widen the server flags bit field as a separate commit, I think
that'll help improve the readability of these diffs and reduce the
length of the diff'll eventually add the cache control feature. OK job@
for the chunks to widen the field.

I'd like to ponder a bit more whether double negation can be avoided and
will come back to that tomorrow. I need to read up a bit more on HTTP
headers to figure out if we're likely to add more config knobs in that
space and if yes, whether those can be made to fit neatly together.
